Did you know that 1 in 4 adults over 65 falls each year - most often at home? A single fall can lead to injury, loss of confidence, and reduced independence. Small, often overlooked hazards can quietly increase your risk.
HARP (Home Hazard Removal Program) helps older adults in Clay County reduce fall risk so they can stay safe and independent at home. A licensed occupational therapist visits your home to review routines, assess safety, and help create a personalized fall-prevention plan with you. This approach addresses both home hazards and daily habits to keep you safe now and over time.
- Who can participate? Clay County residents age 60 or older and enrolled in original Medicare Part B or private health insurance.
- How the program works: OT visits billed to insurance when medically needed. Home Repairs are subsidized by a CCSS grant.
- You get: 2-3 OT home visits in 2 months, safety equipment installed and a follow up visit in 3-6 months.
