TO SIGN UP
INFORMATION NEEDED
It's as
simple as one phone call! We will take all necessary
information over the phone, so there is no need to set up an
appointment. We just need the potential client's basic
information, such as name, address, birthdate, etc. |
RIDES
If the client needs a ride, we will need the date and time
of the appointment and the destination address/phone number.
Transportation must be set up no later than 12 noon the
previous business day. |
OTHER SERVICES
Please be aware that some of our services may be
on a waiting list basis. We provide a variety of services
and programs for Clay County seniors, and we adhere to a
monthly budget for each service. |
